Patch Tests

Your safety always comes first. For services requiring a patch test this must be carried out by Amanda Middleton. It must be performed no less than 48 hours before your appointment. Patch tests performed elsewhere are not valid in any circumstances. 

Booking Fee 

A booking fee may be taken when you book an appointment. This booking fee to secure the specific appointment time and date you have chosen. This amount will be deducted from the treatment cost on the day of your treatment. This booking fee is NON TRANSFERABLE and NON REFUNDABLE. If you choose to cancel or change your appointment this booking fee will be lost*

Payment of this booking fee will be taken as your agreement to these terms.

Cancellation Period 

5 days notice is required to cancel or move appointments.

*In exceptional circumstances, at the discretion of Amanda Middleton, and only if the appointment slot you are vacating can be filled, it may be agreed that the booking fee may be transferred to a future appointment. Booking fees will only be transferred once.

No Shows 

If you miss an appointment with Amanda Middleton a 50% fee will be charged and this will need to be paid prior to rebooking. In addition you may be asked for full payment upfront upon rebooking. 

Gift Cards

Gift cards are valid for 1 year from purchase. They are not redeemable for cash in any circumstanced and cannot be refunded. Gift Cards may not be used to pay for booking fees.  Gift Card T&C’s may be amended at any time.

Training

Training is provided by Amanda Middleton (Amanda Middleton Ltd) using resources provided by 4t Medical. Certification for your course is provided by 4t Medical upon completing the course to the satisfaction of Amanda MiddletonAll booking & payment is made directly to Amanda Middleton Ltd.A booking fee of £125.00 is asked to secure your booking date. This fee is to reserve this date for you and is therefore non refundable and non transferable.The remaining balance is payable 2 weeks before your course date.
